The Derbyshire Home from Hospital Service
The Derbyshire Home from Hospital Service provides practical support for vulnerable people leaving hospital, or those living at home but at risk of being admitted.
You can use the service if you live in Derbyshire, or are registered with a Derbyshire GP, and need more support than is available from family or friends.
Patient Transport Service (EMAS)
East Midlands Abulance Service (EMAS) provides non-emergency patient transport for patients who are registered with GPs in Derbyshire (including the Chesterfield area) and Northamptonshire.
This service is for patients who need medical or clinical support to get to and from their healthcare appointments. Entitlement to transport will depend on whether the patient meets the eligibility criteria set out by the local clinical commissioning group.
Community Transport Derbyshire
Community transport operates across the county providing a variety of services to meet every need and all vehicles are wheelchair accessible. It enables people who are unable to use public transport to get out and about and make journeys most people take for granted. It also provides transport for groups who wish to enhance community activities
Derbyshire Connect provides transport to shopping destinations for people unable to use conventional buses because of age, disability or because they live in areas where public transport is limited.
